MHOA Community Mental Health Service

Community Mental Health Services are organised through three community mental health teams (CMHTs) which operate co terminously with the Primary Care Teams run by the Croydon Primary Care Trust and Croydon Social Services. The CMHTs are made up of all professional groups and operate within a multi-disciplinary framework.

The teams provide assessment, treatment, support and advice to older people and their carers within the community. The service is community focused and seeks to support people within their local community and local networks taking a rehabilitative and enabling approach. It seeks to promote the independence of clients and their carers and encourage healthy, active and fulfilling lives. Services are also provided to residential and nursing homes to support an individualised approach to meeting the needs of residents.

Following the integration, the MHOA Care Management Service was reshaped with two care managers relocated as integral members of each CMHT and managed by the respective CMHT Manager. This provides a more seamless service to the service users and their carers in the Borough.

The service benefits from community mental health support workers also integral to the CMHTs, providing support particularly to people with dementia and their carers. This additional input supports the promotion of independence and assists in the reduction of hospital admissions and other institutional care such as residential care and fits with our aim of improving and joining up health and social care.

Community Care aims to provide health and social care services in the community to enable people to live as independently as possible in their own homes or in other accommodation in the community.
Dementia is characterised by confusion, memory loss, poor problem-solving and poor concentration. There are many diseases that lead to dementia, the most common being Alzheimer's disease. Dementia usually occurs in older people. Dementia in people under 65 is known as early onset dementia. Dementia with Lewy bodies (DLB) is a rarer form of dementia that shares characteristics with both Alzheimer's and Parkinson's diseases. Vascular dementia occurs when cells in the brain are deprived of oxygen. Many symptoms are the same as other forms of dementia, however there are some differences.
Old age consists of ages nearing the average lifespan of human beings, and thus the end of the human life cycle. Euphemisms for older people include advanced adult, elderly, and senior or senior citizen. Older people have limited regenerative abilities and are more prone to disease, syndromes, and sickness than other adults.
