Croydon Reablement Service

The Reablement Service is an alternative to care co-ordination, working with people for a period of 6-12 weeks, with a view to developing lasting resilience and removing or reducing the individual’s dependence on mental health services.

This service does not include prescription of medication and therefore requires that a GP is managing medication prior to commencing. We work with people for 6 to 12 weeks to look at areas of life that they would like to improve, set some goals to work towards and support them to achieve these goals.

We offer 2 initial sessions to help identify goals and then meet weekly to work towards them. We encourage the involvement of friends, family and partners and also offer a separate forum for them four times a year.

Service Definitions

This is the support or care that a person can expect to receive once discharged from inpatient care. Typically a discharge plan will be developed by the multidisciplinary team with the service user which will make clear what care and support will be provided.
