Support Needs Assessment and Placement Service – SNAP

The Support Needs Assessment and Placement service (SNAP) is a central assessment and placement service for vulnerable young people and adults including homeless people, those with a history of offending and people with mental health issues who would benefit from supported housing.

The main aim of supported housing is to develop and sustain a person's ability to live as independently as possible either in their own home or in accommodation with staff on site.

People might need supported housing for a variety of reasons. They may have a mental health problem, a learning or physical disability, be homeless or have a history of offending. They may be a young person who hasn't had the chance to develop independent living skills.
